Below are new patient forms that we need before your first visit to our office. Please download the patient questionnaire and fill it out as soon as possible. You can fax it to our office at 330-762-2988 or mail it to Allergy and Asthma Center of NE Ohio, 215 W. Bowery St., Suite 4500, Akron, OH 44308.

No food in office because of our food allergy patients. Limit people to patient and parent if possible.

If you are being seen for hives, please also fill out the hive questionnaire. Please also attach a copy of your insurance card when you send or fax the questionnaire to us. By getting this information to us ahead of time, it will prevent delays when you arrive for your first appointment

IMPORTANT: Your initial consult may take 1 to 2 hours of time due to thorough evaluation and testing. We would appreciate that you limit the number of people at the visit. Please review the section about medications to stop prior to skin testing. Please do not stop any asthma, heart or blood pressure medications. If you are uncomfortable about stopping any of your medications prior to your appointment, call us.
